DKD:  IMO, the most definitive recent study on Christian Initiation is 
Maxwell Johnson's _The Rites of Christian Initiation: Their Evolution and 
Interpretation_ (Liturgical Press, 1989).  Johnson is Lutheran and is Associate 
Professor of Liturgy at University of Notre Dame.  I studied for a semester under his 
colleague, James White, when I was at Drew University.
