I think you are overlooking all those groups that didn't accept catholic "baptism" as valid baptism and didn't accept the baptism from any group if they were not of like faith and practice, and therefore they baptized all who came to them seeking admission into their church for such groups. This was the practice for centuries in various parts of the world wherever the real church was.

Prior to the 1500's, when the term "anabaptist" was used, it usually
referred to "re"-baptizing someone that was originally baptized by a
heretical sect---say one that didn't believe in the Trinity or denied the
dual natures of Christ. As such, they could not baptize into Christ---much
as the JW's and mormons cannot do so.
After the 1500's those who denied infants can have faith and so only those
who could express their faith verbally should be baptized were given the
moniker. So in the "modern" sense (no infant baptism) they really didn't
exist prior to the 1500's.
